By checking the "acknowledge button" on this application form, the team manager or coach, on behalf of his team, releases the sponsors of the tournament, it’s officials, arena management and all concerned with this event, from any and all liability or any injury or accident which may be incurred by any player or team official while participating in the tournament or while travelling to and from this event and accepts all decisions of the tournament committee as final. All team officials hereby agree to abide with all tournament rules, regulations and decisions and that all registration information and papers are correct.
An OMHA Travel Permit must be completed, and forwarded with registration form to DNMHA Tournament Convenor. A copy with completed Official Roster must be on file with DNMHA Tournament Convenor, before team is eligible to compete in tournament
